The Starter Plan Trap

SimplePractice's Starter plan at $29/month looks attractive, but it excludes the features that over 80% of therapists consider essential for running a practice in 2026:

No Telehealth

Post-COVID, over 80% of therapists offer at least some virtual sessions. Without telehealth on Starter, you need a separate HIPAA-compliant video platform (Doxy.me free tier or $35/month for Zoom for Healthcare).

No Insurance Billing

If you accept insurance, you cannot file claims from Starter. You would need a separate clearinghouse subscription and manual claim generation, adding complexity and cost.

No Secure Messaging

HIPAA-compliant messaging between clinician and client is essential for appointment follow-ups, homework assignments, and session preparation. Without it, you risk HIPAA violations using regular email.

No Treatment Plans

Insurance companies often require treatment plans for prior authorization and continued care justification. Without this feature, you create treatment plans manually outside the platform.

The 2022 pricing restructure moved telehealth out of the base plan and into Essential. Before 2022, all plans included telehealth. The Starter plan now serves primarily as a marketing price anchor. Most therapists discover within the first week of their trial that they need Essential at minimum.

Does SimplePractice offer a free trial?
Yes. SimplePractice offers a 30-day free trial on any plan. You can test all features of the Starter, Essential, or Plus plan without entering a credit card. After 30 days, you must choose a paid plan to continue using the platform.
Is SimplePractice HIPAA compliant?
Yes. SimplePractice is HIPAA compliant and provides a Business Associate Agreement (BAA) to all subscribers. All data is encrypted in transit and at rest. The telehealth video platform is HIPAA compliant. Secure messaging between clinician and client is encrypted end-to-end.
Does SimplePractice support telehealth?
Yes, but only on the Essential ($69/month) and Plus ($99/month) plans. The Starter plan ($29/month) does not include telehealth. The telehealth feature provides HIPAA-compliant video sessions directly within the SimplePractice platform. Sessions can be launched from the calendar with one click.
How does SimplePractice handle insurance billing?
Insurance claim filing is available on Essential and Plus plans. SimplePractice generates CMS-1500 forms and submits claims electronically through a clearinghouse. The built-in clearinghouse charges $0.25 per claim. You can also use Office Ally or other external clearinghouses. ERA (Electronic Remittance Advice) posting is supported for automatic payment tracking.
Can I generate Superbills for clients who self-pay?
Yes. Superbill generation is available on all plans, including Starter. You can create a Superbill after each session that clients can submit to their insurance company for out-of-network reimbursement. The Superbill includes CPT codes, diagnosis codes, and provider information.
What are SimplePractice's credit card processing fees?
SimplePractice uses Stripe for payment processing. The rate is 2.95% + $0.30 per transaction for credit and debit card payments. ACH bank transfers are available at 1% per transaction (minimum $1). There is no monthly fee for payment processing beyond the per-transaction charges.
Can I switch plans after signing up?
Yes. You can upgrade or downgrade your plan at any time. Upgrades take effect immediately and you are charged the prorated difference. Downgrades take effect at the start of your next billing cycle. If you downgrade from Essential to Starter, you will lose access to telehealth and insurance billing features.
